This Discovery Process was designed to provide information and education only. It is not a medical evaluation, medical consultation, treatment recommendation, or determination of candidacy.
Before Proceeding
You will be asked to confirm each of the following before your Consultation Request is submitted:
- I have reviewed the educational materials provided.
- I understand the Discovery Process is for informational purposes only.
- I understand candidacy, treatment recommendations, and medical clearance can only be determined by a licensed medical provider.
- I understand individual results vary and no specific outcomes are guaranteed.
- I understand the Consultation and Good Faith Exam fee is $150.
- I understand the $150 fee will be credited toward the procedure if scheduled within 30 days of the consultation.
- I understand consultations may be completed in person or by telehealth.
- I confirm that I am at least 21 years of age.
